Photo Release Waiver
Authorization and Release
I grant permission to the employees and parishioners of Good Shepherd Episcopal Church to photograph my minor children during church activities and to edit these photographs.
I also grant permission to Good Shepherd to use these photographs in any medium, including print and electronic, without my children's names. If the church wants to publish these photos externally in newspapers, with the names, I will be asked for additional permission.
